Support Planning is a service provided to assist an individual using Consumer Directed Consumer Supports. Support Planning encompasses writing an individual’s Community Support Plan and providing on-going person centered support to implement the Community Support Plan. Support planners work in conjunction with an individual’s case manager in a team approach to identify services that an individual wants and assure those services are provided in a person centered manner. Support planners must pass a support certification test provided by the Minnesota Department of Human services that ensures they are knowledgeable about the Consumer Directed Community Support program. Support planners must re-take and pass the test every two years and are mandated to complete ten hours of training every year.

Provide information about consumer directed community supports and provider options.
Help develop an individuals community support plan.
Research and coordinate purchases and the delivery of services and supports for the individual.
Help solve problems that may occur during the service delivery process.
Help monitor the implementation and expenditures related to the Consumer Directed Community Support Plan.
Assist with recruiting, screening, hiring, training, scheduling, monitoring and paying support workers.
Provide and/or arrange for staff training specific to the individual’s needs.
